In preparation for the mandatory Oct. 1, 2015 transition to ICD-10 CM, we want to remind you that we will need valid ICD-10 codes or detailed clinical information on your orders for pathology and laboratory services. The diagnosis code or clinical history should reflect the reason for ordering the service – the clinical diagnosis or the patient’s signs and/or symptoms that prompted the ordering of the test.
Specifically, beginning on October 1 we will require:
- Lab test orders must have valid ICD-10 diagnosis code(s)
- Pathology service orders must have a narrative diagnosis or valid ICD-10 code(s)
Note that orders placed prior to Oct. 1, 2015 for services on or after October 1, must also use ICD-10. Orders received in our office with insufficient clinical history or ICD-9 codes will result in calls or faxes to your staff for the valid ICD-10 code or more clinical history information so we may identify a valid ICD-10 code. To avoid the disruption to office and billing processes, please send sufficient information with your orders and/or referrals.
